OUR SERVICES
Our services encompass a diverse array of offerings tailored to meet the needs of patients across seven counties. We take pride in serving Otero, Bent, Crowley, Pueblo, Las Animas, Kiowa, and Lincoln, ensuring that we deliver quality support and solutions customized for patients in each community.
Hospice care is designed for patients with life-limiting illnesses, prioritizing comfort and compassionate support rather than curative measures. This specialized form of care is intended for individuals nearing the end of life, as well as their families and caregivers, encouraging an environment of love, concern, and solace during this profound transition.
Patients who opt for hospice care consciously shift their focus from seeking a cure to embracing comfort. Consequently, the primary medical emphasis is on symptom management, which includes effective pain control and ensuring the patient's overall comfort. Hospice aims to enhance the quality of life for individuals in their final days.
The hospice team is a carefully assembled group of dedicated professionals and compassionate volunteers, all working together to ensure that patients nearing the end of their lives receive the highest quality of care and support. At the heart of this team is the medical director, a physician with a deep understanding of the unique needs of terminally ill patients. This individual oversees the medical aspects of the hospice program, ensuring that each patient receives appropriate pain management and symptom control tailored to their specific condition.
Supporting the medical director are the nurses, who play a crucial role in daily patient care. They possess not only clinical expertise but also a profound sense of empathy, allowing them to connect with patients and their families on a personal level. These skilled professionals monitor vital signs, administer medications, and provide education about the illness and the care plan, all while offering emotional support to help patients navigate this challenging time.
Certified nursing assistants (CNAs) are also integral to the hospice team. They provide hands-on assistance with daily activities, such as bathing, grooming, and mobility support. Their presence is invaluable, as they often develop close relationships with patients, offering companionship and reassurance during a time when many may feel isolated or vulnerable.
A spiritual counselor or chaplain is an essential member of the team, providing emotional and spiritual support tailored to the beliefs and values of the patient and their family. This individual is trained to facilitate discussions about end-of-life issues, helping families find meaning and peace during this difficult journey. They may conduct rituals, offer prayers, or simply lend a listening ear, ensuring that the spiritual needs of patients are met with dignity and compassion.
The social worker on the team plays a vital role in addressing the psychosocial aspects of hospice care. They assist families in navigating the complexities of healthcare systems, provide counseling to help them cope with grief and loss, and connect them with community resources for additional support. Their expertise in communication and advocacy ensures that the needs of both patients and families are prioritized throughout the care process.
Finally, volunteers contribute significantly to the hospice team, offering their time and skills to enhance the care experience. They may help with various tasks, from providing companionship to patients and assisting with administrative duties to organizing activities that bring joy and comfort. Their willingness to serve adds an extra layer of warmth and support, creating a nurturing environment for both patients and their families.
Together, this multidisciplinary hospice team collaborates closely, holding regular meetings to discuss patient care plans and ensure that they are meeting the holistic needs of everyone involved. By fostering open communication and a shared commitment to compassionate care, they strive to create a supportive atmosphere that honors the dignity and wishes of patients during their final days, while also offering comfort to their loved ones.
Let’s clarify: choosing hospice care is not a resignation to life’s challenges. Rather, it represents an opportunity to embrace a wealth of meaningful experiences and purpose during one’s final days. It is an invitation to cherish the journey ahead.
Palliative care is a comprehensive approach to improving the quality of life for individuals with chronic conditions. This type of care focuses on providing relief from the symptoms, pain, and stress associated with serious illnesses. It is available to patients at any stage of their disease and can be provided alongside curative treatment. In addition to cancer, COPD, and CHF, palliative care is also beneficial for individuals with other chronic conditions such as kidney disease, Alzheimer's disease, and neurological disorders like Parkinson's disease.
During a home visit, the nurse conducts a thorough assessments to understand the patient's current health status and specific needs. This includes evaluating pain levels, medication effectiveness, and any physical or emotional challenges the patient may be facing. The nurse will then coordinate with the patient's primary care physician and the wider palliative care team to adjust treatment plans as necessary.
In addition to medical support, palliative care often involves counseling and resources for both patients and families. This can include guidance on advanced care planning, such as making decisions about future healthcare preferences, and support groups to help families cope with the emotional aspects of chronic illness. Palliative care aims to empower patients to make informed choices about their care, improve their quality of life, and provide peace of mind to both patients and their loved ones.
Bereavement services are specifically designed to offer support and assistance to individuals mourning the loss of a loved one. At Arkansas Valley Hospice, our bereavement team provides ongoing support to our patients' families through mailers and phone calls for 13 months following the death of their loved one. This outreach aims to help families navigate the emotional challenges of experiencing their first holidays without their loved one.
Importantly, these services are not restricted to the families of our patients; they are available to the entire community, ensuring that anyone in need can access essential support during difficult times.
One of the key offerings is our "Mending Hearts" support group, which meets every fourth Tuesday of the month at 11 AM at the Arkansas Valley Hospice office. This group provides a safe and compassionate space for participants to share their experiences, find comfort in community, and begin the healing process together.
In addition to the monthly support group, we also host a 6-week grief workshop twice a year, specifically in the fall and spring. These workshops offer deeper insights and coping strategies for navigating grief, along with opportunities for participants to connect with others who are experiencing similar challenges.
We encourage anyone in the community who is dealing with loss to take advantage of these valuable resources.
Volunteers are truly the heart of our organization, playing a vital role in everything we do. Their dedication and compassion help us fulfill our mission and make a meaningful impact in the community. We offer a variety of volunteer opportunities to suit different interests and skills.
Volunteers can get involved in patient care, providing essential support and comfort to those in need. We also have opportunities in grief support, where volunteers can lend a listening ear and help individuals navigate their emotions during difficult times.
For those who prefer a more behind-the-scenes role, we have administrative duties that keep our organization running smoothly. Additionally, volunteers can participate in fundraising activities, helping to raise the necessary resources to sustain our programs.
Special events provide another exciting avenue for volunteers to engage with the community and raise awareness about our cause. We also offer opportunities to support veterans, ensuring they receive the care and recognition they deserve.
No matter where you choose to contribute, your involvement as a volunteer is invaluable to us and makes a significant difference in the lives of those we serve.
Hospice care for veterans represents a crucial service that recognizes the unique experiences and needs of those who have served in the military. Organizations such as "We Honor Veterans" play an instrumental role in delivering specialized care and support tailored to veterans and their families.
This type of care adopts a holistic approach to end-of-life support, addressing not only physical symptoms but also the emotional, spiritual, and psychological well-being of veterans. Importantly, hospice care extends to the families of veterans, offering resources and support for caregivers. Available services may include counseling, educational resources, and guidance in navigating the complexities of the healthcare system, which can be particularly daunting for veterans' families.
Collaboration with the Department of Veterans Affairs (VA) ensures that veterans receive the benefits and services to which they are entitled. This coordination streamlines access to healthcare, financial assistance, and other veteran-specific resources. Furthermore, our social worker is dedicated to creating opportunities for veterans to reflect on their experiences, share their stories, and establish lasting legacies. Engaging in activities such as life review sessions can significantly aid veterans in attaining peace and closure.
Overall, hospice care for veterans is designed to deliver compassionate, respectful, and personalized support during a critical period in their lives, ensuring they receive the dignified care they deserve as they transition from life.
Our companion care program is designed to offer support and companionship to individuals who may be experiencing loneliness, isolation, or difficulties related to aging or health issues. These programs typically involve trained caregivers who provide not only physical assistance but also emotional support, social interaction, and engagement in various activities.
The primary objective of a companion care program is to enhance the quality of life for participants. This service is particularly beneficial for seniors living alone, individuals recovering from illness, or those with disabilities who may find it challenging to engage in social activities. Caregivers in these programs are trained to understand the specific needs and preferences of their clients, enabling them to deliver personalized care that fosters a sense of belonging and purpose.
Engaging in conversations, sharing stories, or simply spending time together can significantly alleviate feelings of loneliness and depression. Caregivers may encourage clients to participate in hobbies they enjoy, such as gardening, arts and crafts, or playing games, which can stimulate mental engagement and creativity.
While the primary focus is on companionship, caregivers can also assist with light housekeeping and meal preparation. By prioritizing companionship and support, these programs not only enhance the quality of life for participants but also provide peace of mind for their families.
